East Midlands Dyslexia Network Privacy Notice
Who we are
East Midlands Dyslexia Network operates to provide assessment services for those with, or who suspect they may have, specific learning difficulties, as well as provide workplace needs assessments, workplace coaching, and individualised learning plans (based on the findings of the diagnostic assessment).
East Midlands Dyslexia Network take your privacy seriously. In this Privacy Policy, we’ve provided lots of information on when and why we collect your personal information, how we use it, the limited conditions under which we may disclose it to others and how we keep it secure.
In this Privacy Policy, references to ‘we’ or ‘us’ refer to East Midlands Dyslexia Network. References to ‘you’ mean you and/or the child or young person who whom you have responsibility.
Information you provide:
This is usually information provided by you on the questionnaire that you complete prior to assessment. This may include contact details (such as name, address, telephone number and email address), as well as data such as date of birth, native language and whether you are a UK citizen.
It may also include sensitive personal data (such as physical/sensory needs, health, cognitive concerns, why you are seeking an assessment, details of a disability or specific learning difficulty, qualifications, support you may have had in the past, assessment data and notes written during the assessment process).
It may also include any previous reports or medical documentation that you provide.
For adults, it may include name of employer and job role, any support provided in work, strengths/challenges in relation to work and your current work situation.
For children, this may include school details, any previous assessment details and previous/current support.
Information from third parties:
We may ask for additional information from a school/college/ employer and video call/face to face meetings to enable us to provide you with the most appropriate assessment service. This would only be with specific written (usually emailed) consent from you.
Why do we collect and use your information?
Your personal information is collected to enable us to plan and provide the assessment service that you have requested (including invoicing purposes).
When using your personal data, we rely on two lawful bases:

consent (where you have given clear consent for us to process your personal data for a specific purpose, ie a diagnostic assessment for specific learning difficulties, in accordance with this Privacy Notice), and
-
contract (whereby you have entered a contractual agreement with us to undertake an assessment for you).
Who do we share your personal information with?
If an employer has requested your assessment (and has paid for it), we will provide them with a summary report. In this instance, personal information would include contact details, diagnosis of a specific learning difficulty (if given), strengths and challenges and recommendations. It does not include sensitive data such as test scores.
If you have requested a learning/coaching plan post assessment, then a copy of your diagnostic report, along with certain test papers, will be provided to the specific named person agreed with you (usually a Specialist Teacher).
If we are under a duty to disclose/share your personal data to enable us to comply with any legal obligation (eg law enforcement agencies), moral obligation (eg duty of care/safeguarding) or to protect the rights, property and safety of East Midlands Dyslexia Network, we will do so.
Access to your personal information
Under the GDPR, you are entitled to view, amend or delete the personal information that we hold. Our Data Protection Officers are Justine Webb and Lisa Brockway. Please email your request to Justinewebb73@hotmail.com or lisa.brockway1@gmail.com. Please note that we will need you to prove your identify with two pieces of approved documentation.
Our standard practice is to keep records of your assessment materials for one year, when they are then disposed of securely. Your assessment report will be kept electronically (in a secure format) usually for 5 years.
By submitting your personal data, you agree to the transfer, storing and processing of it. East Midlands Dyslexia Network will take all reasonable steps to ensure that your data is treated securely and in accordance with this Privacy Notice.
Note: Requests from you to delete data will be considered on a case-by-case basis but we may be unable to remove all assessment records for legal reasons.
Changes to this Privacy Notice
This Privacy Notice will be reviewed annually.
